This position will work in both Cardiopulmonary Rehabilitation and Cardiopulmonary Clinics as needed. This is a PRN (as needed) position with shifts Monday through Friday from 0745-1715. The RN shall act as the patient liaison with the visiting cardiologists, assisting with triages, medication orders and refills, and DME set up. In Cardiopulmonary Rehab, the RN shall be responsible for assisting patients with cardiac and/or pulmonary rehab following national AACVPR certification guidelines.
Responsibilities:
- Provide direct patient care to Cardiopulmonary Rehab and Specialty Clinic patients.
- Initiate assessment and care plan based on the patient’s presenting problems and the provider’s medical diagnosis.
- Collaborate with other healthcare team members regarding treatment planning (healthcare team members include, but are not limited to, pharmacists, dieticians, physical therapists, and respiratory therapists).
- Keep providers and healthcare team members informed of patient’s condition and rehab status.
- Work well with visiting cardiologists and pulmonologists.
- Perform required testing associated with Cardiopulmonary Rehab and Clinic.
- Communicate effectively between patient, patient’s family, providers, and other staff members of the healthcare team in evaluation of care to ensure continuity of care.
- Collect, evaluate, analyze, document, and prioritize pertinent patient data using the full range of patient needs – physical, psychosocial, spiritual, educational, and discharge.
- Follow provider orders and review patient information to assess patient condition.
- Determine requirements for treatment and/or procedure such as type and duration of exercise and education.
- Assess tolerance of activity levels, collaborate with other team members, and make recommendations/referrals regarding appropriateness of return to work or ADLs.
- Complete ongoing patient assessments as determined by patient condition.
- Provide information to family and/or significant individuals in assessment activities and patient progress and condition.
- Perform initial assessments and initiate communication with healthcare team members regarding treatment planning; i.e. nutrition, psychosocial, medications, etc.
- Collaborate with staff and appropriate resource persons to formulate and update individual treatment plans (ITPs) to work toward positive patient outcomes.
- Possess a strong knowledge of arrhythmia interpretation and cardiopulmonary risk.
- Follow Cardiopulmonary Guidelines set forth by CMS regulations and the American Association for Cardiovascular and Pulmonary Rehabilitation.
- Schedule, document, and charge appropriately in electronic medical record and LSI system.
- Maintain and review authorization and scheduling work queues in electronic medical record system.
- Cross train and supervise Pulmonary Rehab exercise sessions if needed.
- Collaborate with Cardiopulmonary Clinic staff on prepping Cardiology and Pulmonology Clinics.
- Demonstrate competency in performing Cardiac Testing (i.e. Stress Testing, EKGs, Pacemaker Checks, Blood Pressure, SpO2 Checks, etc.).
- Initiate and run intravenous medications during cardiac testing.
- Assist the cardiologist during cardioversions and TEE procedures.
- Assist patients and providers with medication refills, prescription entry, e-scripts, and DME set ups.
- Clean clinic instruments and equipment and maintain stock of supplies (i.e. crash cart monthly checks).
- Complete clinic encounters and enter charges appropriately in electronic medical record.
- Perform daily charting, department billing, pre-authorizations, medical necessity, and chart audit adjustments per hospital standards.
- Other duties as assigned by management.
Requirements:
- Graduate from an accredited Registered Nurse Nursing program.
- Prefer one year of clinical experience. New graduates considered.
- License as a Registered Nurse in the State of Nebraska.
- Certifications: Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Cardiac Life Support/Pediatric Advanced Life Support (PALS). If certifications are not current, RN shall participate in classes offered at SRMC.
